A certified copy of a birth certificate for a person born in Troy, NY can be obtained from the Vital Records office. The office is located in Troy City Hall at 433 River Street. Who is eligible to obtain a birth certificate copy?
Birth records can only be released to:
- The person on the birth certificate if over 18 years old
- A parent listed on the birth certificate
- A lawful representative or by court order
You must have proper identification to obtain a certified copy of your birth certificate or the birth certificate of your children. A certified copy of a court order is required for children in your custody.
Birth and death records are NOT public records and their confidentiality is protected by New York State Public Health Law. Vital Records are not subject to the provisions of the Freedom of Information Law (FOIL) and are not subject to public inspection.
How to request a certified copy of a birth certificate
A person born in the City of Troy, NY may request a copy in person or mail a completed request form form to:

If mailing the request, please include a copy of your ID and the required fee.
Information required to obtain a birth certificate
- Name at birth
- Date of birth
- Father's name
- Mother's maiden name
- Relationship of person making request
- Purpose certificate is being requested
- Driver license
- Non-driver license
- Employment ID
The fee is $10 per copy. Total for one copy is $10. Total for two copies is $20, etc.
